Medical Forms

Please take the time to read below to see what medical forms are required for your camper.

Medical Release Form: This form is required for all campers.

Food Allergy Action Plan: If your child has any food allergies, you and your child’s physician or authorized health care provider will need to complete the Food Allergy Action Plan.

Asthma Action: If your child has been diagnosed with asthma, you and your child’s physician or authorized health care provider will need to complete the Asthma Action Plan.

Physician's Medication Order Form: If your child will need ANY prescription or over-the-counter medication during camp hours, we will need this form completed by you and their physician or authorized health care provider.  No medication (topical or oral) will be administered without the completed form and medication.

Along with the form, a new bottle/box of the prescription/over-the-counter medication must be provided - one child per box. The information on the prescription label must match the information on the Medication Form/Physician’s Order Form to be accepted.
