We have tried to make our application process as simple and straightforward as possible, but we also know that many of you will have questions along the way. Please do not hesitate to contact the school if you have any questions.
Parent Application
Please complete, sign and return the application form to the school along with a non-refundable fee of $100 for Pre-Kindergarten and Kindergarten or $300 for all other grades. The receipt of the complete application and fee initiates the formal application process.
Student Application
Please complete the applicable sections.
Pastor Recommendation
Please complete the parent section of the pastor's recommendation form and forward it to your church pastor. Please request that the pastor return the completed form directly to the Office of Admissions as soon as possible using the enclosed envelope.
Teacher Evaluation
Enclosed in the application package is a teacher evaluation form for applicants to Pre-Kindergarten and Kindergarten, and another evaluation form for all other applicants. Please request that the teacher return the completed form directly to the Office of Admissions as soon as possible using the enclosed envelope.
Transcript
Enclosed in the package is a transcript request form to be completed by your child's current school. Please request that the school send the completed form and your child's academic records directly to the Office of Admissions as soon as possible.
Testing
Applicants to Pre-Kindergarten and Kindergarten undergo an in-house assessment. Students applying for Grades 1-5 are asked to complete the Woodcock Johnson III Tests for Achievement and Cognitive Abilities. Students applying for Grades 6-8 are asked to complete the Woodcock-Johnson III tests as well as the TOWL-3 Test of Written Language. Please contact the Office of Admissions to schedule a date for testing.
Classroom Visits and School Evaluation
The prospective student may be asked to join a grade-appropriate class for one or two sessions when an application is received during the school year. The Office of Admissions will contact you to schedule a classroom visit.
Financial Aid (if applicable)
The Fourth School utilizes the Independent School Management Financial Aid for School Tuition (FAST) Program in Wilmington, DE. FAST provides Fourth School a need-based financial aid analysis service which ensures that our process for establishing financial need adheres to nationally established standards of best practice. All information from FAST is kept confidential, as is all information pertaining to a family's financial aid request.
Fourth Presbyterian School requires that all new families interested in applying for financial assistance visit the FAST website to complete the FAST application. FAST also asks that you mail copies of your 2011 1040 and W-2 forms to them directly and to the Fourth School Business Office as well.
FAST will send a Report of Family Contribution to the Business Office once you complete your online application and they are in receipt of your 2011 tax information. The Financial Aid Committee will meet and families will be notified of their financial aid awards.
All financial aid recipients must inform the Business Office of any substantial change in income, as we may adjust your grant in accordance with this change.

Admission Decisions
All applicants will be notified by letter on a rolling basis once the application process is complete.
